I received something like this in the past, I liked the eye mask better than the one I bought from walmart because the elastic was better, but it also came with ear plugs and an ear plug case – I think this was something that was mailed in from the paper. I hope I’m in one of the first come, first serve.

I was able to send the form out by reading the privacy policy, click out of the link when you’re done, and then click send again.
