There is a peelie or hanging tag on the product it self with the rebate form.
Please note the rebate form does day the purchase must be made 3/1/12- 9/1/12. Im not sure if they will honer it or not if your reciept is dated before then

I called the rebate center # on the form: (1-800-619-4703.) They said to disregard the 3/1 buy date. There are also multiple products. You can only submit 1 offer per name / household / address despite them being different products. Allow 6-8 weeks for the retund.
The print on the tags & peelies is teeny tiny!! Be sure to take your glasses if you wish to read it before buying. I didn’t.
