Do you know if you can use it if you already used one last time?

Yes you can! They are completely different bar codes. I just used a new one today (printed on the same computer as the previous one was printed on).

How do I get to the coupon? When I click on the link, it’s a CVS page with Medicare info at the bottom, but not a survey. The top of the page is blank.

    The one I used last week didn’t scan–just a heads-up.

You should see a button that says “Start Quiz Now” that you need to click on.

This didn’t scan for me either today. The manager let me take the $5 off, but she said there was something wrong with the barcode.

The barcode would not scan today at CVS and there is not a code on it, so I wasn’t able to use it.

I couldn’t use mine either, the barcode wouldn’t scan and the poor checkout lady tried several times to enter it manually, but no luck.
