For the Whitestrips, the website states it is valid from 9/30/12 to 12/29/12, but the actual form states 7/29/12 to 9/29/12. They probably forgot to upload a new form. I would wait for the new form before I send it in.

You’re right Bridget. I saw it in the Sunday SmartSource with those dates, but saw the old form. Was hoping Crest would have updated the form today since it was in the Sunday newspaper yesterday, but I guess they haven’t yet.
