I got a place to enter my email address and my friends' email addresses, but nothing to enter my mailing information. Did I miss something?

I tried to send it to my other email address just to see how it would work and I never received an email. But my mom sent in my email address and I got an email from Quaker and put in my mailing info and it said the sample would be sent.

Same thing it didn't ask me for my information on where to send it. All it did was say thank you. Maybe it'll show up in my inbox.
