Mine also says “failed to submit form” I tried a few times and made sure everything was filled out right. Hmmm….

I got that several times, finally closed out completely, clicked the link here again, filled out the form again and got a “Thank You” this time. Maybe just keep trying??

Read the fine print. They will only send you a sample if you check the box to receive future emails. I didn’t check the box, then read after the fine print…so, I’m not expecting to receiving anything now! :(

I filled out the form and didn’t get a thank you message or anything. I also didn’t check the box for future information. I just went back to the site because I figured it didn’t go through since I received no message and filled out the form again and got a message that I can only request One sample per day. Guess I will try again tomorrow night and see if it goes through. But that makes it sound like you can request more than one.

After you hit submit the page doesn’t change. It just has a small yellow thank you on that same page. Easy to miss it and at first I thought it had failed for me too. Good luck.
