Yes, you are correct. They will only honor the original form, not printed copies. Once again, Tara is misleading.

Thanks Wendy!! I wish I’d thought of that. : ) FYI: The form I printed wasn’t the same one I saw on DSM. This one though does say it’s an original form. I wonder if Tara just inserted the incorrect link? I also tried to print out that same form & it wouldn’t even on the Cascade sight.
