I am not seeing anything about a free sample.
You have to click toward the LH top side of the bottle and also make sure pop-up blockers are disabled. However, I received a message that says this sample is no longer available :-(
NO SAMPLES!
It said sorry, no more samples. check back later for maybe more samples. Idiot Sam's!
I checked earlier today and again now, and there was never any link for a free sample...did anyone get one?

There definitely were samples because I test all of the sample offers before I post them and I was able to request one. There was a spot on the bottle that you had to click on to get to the form. Sam's Club periodically restocks their samples, so I'll let you know if I see them available again.
