I'm not sure what I'm doing wrong. When I go to the site, the fill in form starts just to the left of my screen. I can't access the first 8 or 9 letters, so I can't make out what is needed in each blank. Are there some settings I need to change?

Is there a reason why you need a cell phone number? My sis in law is submitting this for my niece, and she doesn't have one. She put in her home phone instead, but if there is some sort of text confirmation, she wont get it.

there's just a little circle that spins and spinsits like its loading but no form... i turned the pop up blocker off, and its the same deal...

I didn't get a confirmation, but a little box did pop over the form that said "Thanks for submitting" or something to that effect.

What happens after you order it? Is there a confirmation page? I just filled out the form and when I hit submit, it went back to an empty form.
