Woohoo . . . the $5/$30 CVS Coupon for taking a short, 3-question survey on Medicare Part D is back! The coupon expires two weeks from printing.
Check out this week's CVS deals to see how you can put this coupon to work -- or better yet, save it for the hot Black Friday deals later this week!

Do you know if you can use it if you already used one last time?
Yes you can! They are completely different bar codes. I just used a new one today (printed on the same computer as the previous one was printed on).
How do I get to the coupon? When I click on the link, it's a CVS page with Medicare info at the bottom, but not a survey. The top of the page is blank.
The one I used last week didn't scan--just a heads-up.
You should see a button that says "Start Quiz Now" that you need to click on.
Mine would not scan either but the register person stillgave me five off
This didn't scan for me either today. The manager let me take the $5 off, but she said there was something wrong with the barcode.
The barcode would not scan today at CVS and there is not a code on it, so I wasn't able to use it.
I couldn't use mine either, the barcode wouldn't scan and the poor checkout lady tried several times to enter it manually, but no luck.
